Dear patients:

For your convenience, our clinic offers the following services:
1) Blood draw
2) Immunizations

You do not need to see the doctor for the above stated Services. These Services are considered "Non-MD" visit and are billed under the Non-MD CPT Code 99211.

As a courtesy to you, we will submit these claims to your insurance company. Nevertheless, please he aware that your insurance may or may not pay for these services. It will be your responsibility to pay for these services If they are not covered by your insurance company.

Furthermore, you will also be responsible for any Co-pays. Co-pays apply even if you did not see the doctor for these services.

You may contact your insurance company prior to obtaining these services to find out if these procedures will be covered or whether you will be responsible to pay for them.

If you choose to go an outside lab such as Quest, please let us know. The cost of the services will be determined by that facility and your insurance company.
